Brockton Adult Foster Care is a contracted MassHealth provider delivering high-quality, community-based care that enables seniors and individuals with disabilities to live with dignity, independence, and comfort in a home setting. Our program offers a trusted alternative to institutional facilities and residential group homes, allowing members to receive personalized care in the safety and familiarity of their own homes. We provide comprehensive, ongoing case management led by a registered nurse care manager and a licensed social worker care manager. Brockton Adult Foster Care also supports family members and caregivers through professional training, essential resources, and a monthly financial stipend to help ease the demands of daily caregiving.
